Please review the following terms before booking an appointment or using this website. They set out expectations for care, communication, and the responsibilities we share as a clinic and as your care team.

  • This site is not for emergencies — call 911 if you need immediate help.
  • Give us 24 hours notice to cancel; repeated no-shows may incur a fee.
  • Some services (forms, notes, travel) are not covered by OHIP.
  • Do not send urgent clinical concerns by email or web message.

Acceptance of these terms

  • By accessing this website or receiving services from Eclat Medical Centre at Downtown Toronto, you agree to be bound by these Terms & Conditions.
  • If you do not agree, please discontinue use of the site and contact our administration team before proceeding with any appointment.
  • We may update these terms from time to time; the most recent revision date will always appear at the top of this page.

Nature of our services

  • We are a licensed family medicine clinic in Ontario providing scheduled, walk-in, and virtual care where appropriate.
  • Our services are NOT an emergency service. If you are experiencing a life-threatening situation, call 911 or go to the nearest emergency department.
  • Health information on this website is provided for general educational purposes and is not a substitute for professional medical advice.

Appointments, cancellations, and no-shows

  • Please arrive 10 minutes before your appointment with your health card, medication list, and any relevant records.
  • We ask for at least 24 hours notice to cancel or reschedule an appointment so we can offer the slot to another patient.
  • Repeated missed appointments may result in a no-show fee for uninsured visits, and, in extended cases, a review of the patient-provider relationship.

Fees for uninsured services

  • Certain services are not covered by OHIP (for example: sick notes, form completion, third-party medicals, and travel consultations).
  • A schedule of uninsured service fees is posted at the clinic and available on request; payment is expected at the time of service.
  • We accept debit and major credit cards. Receipts for insurance or tax purposes are issued upon request.

Prescription renewals and referrals

  • Renewals for regularly prescribed medications require an appointment or a phone/virtual visit at our discretion.
  • Referrals to specialists are made based on clinical need and available referral pathways; wait times are outside of our control.
  • Please allow 5 business days for routine paperwork, form completion, and referral processing.

Communication and electronic messages

  • Non-urgent messages may be sent by phone or through our secure email channel; do NOT send urgent clinical concerns by email.
  • By providing your contact details, you consent to receive appointment reminders and care-related communications.
  • You may unsubscribe from optional newsletters at any time using the link in each email.

Patient responsibilities

  • Provide accurate, complete health and contact information and update us when it changes.
  • Treat clinic staff, providers, and other patients with respect; abusive or threatening behaviour will not be tolerated.
  • Follow agreed-upon care plans and let us know promptly if your condition changes or if you cannot follow the plan.

Website use and intellectual property

  • All content on this website — text, images, logos, and design — is the property of Eclat Medical Centre or its licensors and is protected by copyright.
  • You may view and print pages for personal, non-commercial use; any other use requires our written permission.
  • You agree not to attempt to disrupt, reverse-engineer, or gain unauthorized access to any part of the site or its systems.

Limitation of liability

  • To the fullest extent permitted by Ontario law, Eclat Medical Centre and its providers are not liable for indirect or incidental damages arising from use of this website.
  • Nothing in these terms limits liability for professional negligence or any liability that cannot be excluded under Ontario law.
  • Third-party links on this site are provided for convenience; we are not responsible for the content or practices of external sites.
